APPLICATION PROCEDURE
  1. Pay the Admission Processing Fee at the Accounting Office.
  2. Proceed to Admissions Office and present the official receipt of payment to secure your (1) Application Form; and (2) Test permit.
  3. The Admissions Office will give you the schedule of the Admission or Entrance Test.
  4. The Admissions Office will advise you of the release date of entrance test results.
  5. Upon receiving your Notice of Acceptance, you may proceed to pay your reservation.
WHAT TO SUBMIT
  1. Original and Photocopy of report card (Form 138).
  2. Certified True Copy of PSA Birth Certificate.
  3. Form 137

  4. For those who stopped schooling: an original certification from the Senior High School Registrar clearly stating that the student has never enrolled in any college.



NOTE: The FIRST-COME, FIRST-SERVED policy will be strictly observed.

The reservation fee is non-refundable and non-transferable but will be deducted from the fees upon enrolment.

  2. If qualified, please pay the Admissions Processing Fee at the Accounting Office.
  3. Present the official receipt of payment to the Admissions Office to secure application form.
  4. The Admissions Office will provide you your schedule for grade evaluation and interview with the Dean/Chair of the College where you wish to enrol.
  5. The College Dean/Chair will decide on the subject/s to be credited and that need/s validation and shall recommend you for admission.


    4. If necessary, the College Dean/Chair may recommend that you take the Admissions Test.
    5. Fill out the Request-for-Credit Form for approval by the Dean. The validating exam form is available at the accounting office. The form is presented for the Dean’s approval and for the assignment of test schedule. The Dean/Chair will submit the test result to the Registrar who will apply the proper grades credit. A validating exam fee must be paid before taking the exam.
    6. Secure and submit the credited subject(s) form, duly signed and approved by the Dean at the Registrar’s Office.

  3. Transfer Credentials/ Honorable Dismissal

Notes:

  • Acceptance of returnees shall depend on the available slots in their respective course and year level.
  • The number of units to be enrolled by second coursers shall be determined by the College Dean/Chair.
APPLICATION GUIDELINES

The following guidelines apply to crediting minor subjects taken in the previous school:

  1. Minor subjects taken in the last school attended will be given credit provided that these have the same course descriptions and number of units, and a passing grade of 75.
  2. If the subjects do not consist of the same number of units,they are given credit only if they carry more units than what is prescribed by FAITH.
  3. Subjects taken in previous school, will not be given credit if these have prerequisite/s based on First Asia Institute’s curriculum.
