Request of document(s) may be availed by:

  • Currently enrolled students
  • Graduates/Alumni
  • Existing students cleared of their financial accountability and with complete credentials
  • Payment must be done online when requesting for online documents for easy monitoring

How to Request for Documents

1. Click the appropriate Google form link

 

2. Fill out the Google Form completely and accurately

  • Kindly ensure that your contact details are correct, updated, and lines are always open
  • For verification and confirmation, we will send you an email regarding your request and amount to pay

3. Claim your documents at the Registrar’s Office

  • Documents can be claimed at the Registrar’s Office 5 working days upon receipt of payment
  • Office hours is from 8:00 AM to 4:00 PM, Mondays to Fridays

In case you cannot personally claim the documents:

  • Send an authorization letter together with the proof of payment and any valid Government ID from your authorized representative
  • Unclaimed documents after 60 days from the date of release shall automatically be shred and disposed

4. Check your E-mail

  • You will receive a PDF scanned copy of your requested document through email upon request

Please send the following to college-accounting@firstasia.edu.ph and indicate if for Enrollment or for Outstanding Balance.

Any and all unused non-school fees shall be credited to the student’s account.
